How to Order

• On the order form clearly print your name, address, zip code and phone number.

• Check method of payment. check or money order, credit card.

• Write in the appropriate space the credit card name, such as Master Card, Visa, Discover etc, the card number, its expiration date and card holders signature.

• List the plans in the space provided. Make certain to include the letter designation for each plan. These letters are important - without them we cannot properly process your order.

• In the space provided write in the cost for each drawing you are purchasing.

• Total the cost of the plans

• Add the Shipping and Handling Cost

• Add .05 % Massachusetts Sales tax to the Sub Total

• Enter the Total Amount Due
